Patients who have a chronic medical condition, as determined by their GP, are eligible for Medicare rebates for certain allied health services on referral from their GP.
What conditions must a patient have to be eligible for a Chronic Disease Management Plan?
To be eligible for any of the Chronic Disease Management Plan, a patient must have a chronic medical condition, as determined by their GP. This is one that has been or is likely to be present for three six months or longer and includes but is not limited to; asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.
Is there an age restriction for a Chronic Disease Management Plan?
